Fire & Ambulance Reports

The City of Edwardsville is able to provide reports for fire incidents and ambulance service. Requests must be submitted in writing and contain all pertinent information. Reports are free for the patient (ambulance service) or property owner (fire incident), but must be picked up in person (Photo ID is required) at the Edwardsville Fire Department, 333 S. Main Street. All other requests for reports are subject to a $5 processing/photocopy fee.

Reports will not be released until payment has been provided (if applicable). Checks (payable to "City of Edwardsville") can be submitted with the written report request or dropped off or mailed to:

Ambulance and/or Itemized Billing Report

Requests must be submitted in writing; please include:
  • Specify whether you are seeking an ambulance report, itemized billing or both
  • Patient's name
  • Date of service
  • Incident number (if available)
  • Patient's signed HIPAA release form
  • Where report should be sent

Fire Report

  • Specify that you are requesting a fire report
  • Address of fire
  • Date of fire
  • Incident number (if available)
  • Where report should be sent

Ambulance Service Payments

Payments for ambulance service can be made quickly and conveniently using PayGround, our online payment system. They also can be made in person in the Finance Department at City Hall using credit/debit cards, checks (payable to "City of Edwardsville"), money orders or cash. Payments also are accepted by phone by contacting the Finance Department at 618-692-7500.
